Immunization Records
There are three ways to send your immunization records from home to Student Health:
- Log in to your Gateway Patient Portal and select Medical Clearances to upload your Immunization record.
- email - as an attachment, or
- fax: 805-893-3593
When choosing fax or email you must include:
- full name
- perm number
- date of birth
